Official Rules

- Sponsor. Dental Makeup, Inc., doing business as Neer, 2200 Colorado Ave, Suite H, Santa Monica, CA 90404 ("Sponsor"). Dr. Avi Patel is a paid partner of Sponsor. This giveaway is not sponsored, endorsed, administered by or associated with Instagram or Meta.
- Eligibility. Open only to dentists who hold an active, unrestricted dental license in the United States and currently practice in the US, and who are 18 or older at the time of entry. Employees of Sponsor, Levv & Jane, and Dr. Avi Patel's practice and media team, and their immediate family and household members, are not eligible. Void where prohibited or restricted by law, including where a dental board or employer prohibits acceptance.
- Entry period. Begins when the giveaway post is published on Dr. Avi Patel's Instagram account and ends at 11:59pm Pacific Time on the 14th calendar day after ("Entry Period"). Sponsor's clock is the official timekeeper. Entries received outside the Entry Period are void.
- How to enter. Fill out the entry form at getneer.com/avi-giveaway during the Entry Period. You can get the link by commenting CASE on Dr. Avi Patel's giveaway post, or go to the URL directly. Commenting alone is not an entry. Limit one entry per dentist. Extra or automated entries void all of that person's entries.
- Prize. One winner gets: one full Neer prepless veneer case (up to 10 units) for one patient; one Moment trial smile for that same patient; an onboarding training session with Neer's clinical team; and one virtual case planning session with Dr. Avi Patel (by video, up to 60 minutes, scheduled at a mutually agreed time before the case is seated). Approximate retail value: $1,500. The case planning session has no cash value. If Dr. Avi Patel is unavailable, Sponsor may substitute a session with a member of Neer's clinical team. The winner covers shipping, their own scans, chair time and all other clinical costs. The prize is for the winner's own practice. It can't be transferred, sold or exchanged for cash. Sponsor may substitute a prize of equal or greater value if needed. The case has to be ordered within 90 days of winner confirmation or the prize is forfeited.
- Winner selection. Within 3 business days after the Entry Period ends, Sponsor will pick one winner by random draw from all eligible entries. Odds depend on the number of eligible entries received.
- Notification. The winner will be notified by email within 3 business days of the draw. They then have 5 business days to reply and verify their license. If they don't respond or don't qualify, an alternate winner is drawn. The winner will be announced on Dr. Avi Patel's Instagram after verification.
- Taxes. The winner pays any taxes on the prize. Because the prize is valued at $600 or more, the winner must provide a completed IRS Form W-9 before the prize is fulfilled, and will receive an IRS Form 1099.
- Clinical responsibility. The winning dentist is solely responsible for patient selection, informed consent, treatment and outcomes. Sponsor provides product and training only and makes no guarantee of clinical results.
- Publicity. Unless prohibited by law, entering gives Sponsor and Dr. Avi Patel permission to use the winner's name, practice name, city and state to announce the winner. No patient images or information will be used without separate written patient consent.
- Privacy. Information you submit is collected by Sponsor. Entrants agree that Sponsor may contact them about Neer products, and they can unsubscribe at any time.
- General. Sponsor may cancel, change or suspend the giveaway if fraud, technical failure or anything else beyond its control affects it. By entering, entrants release Sponsor, Dr. Avi Patel, Levv & Jane and Meta from any liability connected with the giveaway or the prize. These rules are governed by California law.
- Winner's name. For the winner's name after the draw, email patrick@getneer.com with the subject "Avi Giveaway Winner" within 30 days of the announcement.
